If you‘re looking to make a referral for someone in your network, please use the directions below to get your request to us as soon as possible. For Providers & Mental Health Professionals

If you’re looking to expedite the intake process for your patient or client, please download our Referral Form to help speed up the process prior to their arrival. If you would like to request pre-approval for your patient or client, please follow the three steps below:

  1. Call Michiana and ask to speak with the Intake Department (or a nurse supervisor if/when intake is not available) to inform them that we should expect a fax.
  2. Include the pre-approval form to ensure all proper information is conveyed, including any lab work that was performed and the patient’s demographic sheet.
  3. We want to make sure that we are receiving your information in real time and able to diligently respond in a timely manner. Please fax the information to our Intake Department at 574-936-6833. If an additional number is needed, you may then fax to our nurse’s station at 574-941-5733.

Our Assessment and Referral specialists are available 24 hours a day, seven days a week to schedule a no-cost, mental health assessments. Call 800-795-6252 or visit our facility to get started. For questions about our programs, you can also use our online contact form. In the case of a medical emergency or crisis, please dial 911 or go to the nearest emergency room.
